This presentation will focus on what is considered appropriate documentation specific to a patient’s condition, the rationale for treatment, testing, etc., the patients’ understanding of the plan of care, and their documented response.
Additionally, this presentation will focus on having a reliable tracking system to follow up on patients to ensure patients are receiving the necessary care and are not lost during the follow up process.
